CDF Technical Incident Support Form
This Technical Incident Submission Form allows you to report a technical issue you are experiencing with the CDF application form.
If your inquiry is related to application guidelines or instructions (how to respond to one of the questions, the evaluative criteria, applicant eligibility, etc.) please do not submit this form, but email your program officer directly. New applicants who do not yet have an assigned program officer can email those questions directly to cdfhelpdesk@culture.nyc.gov.
If your issue relates to registering a new user or organization, please make sure to review the Registration Help Guide on the DCLA website for commonly asked questions.
Complete the form with as much detail about the issue you encountered as possible.
You can expect to receive a response to inquiries submitted on this page within (2) business days of form submission. Please do not submit the same inquiry multiple times.
